If I understand your requirements correctly, the following should help you. It is not pretty, but it works.
Put this html in the AwareIM folder and give you users a link to this form.

Additional information and required changes:
logonGuest.aw => Accessing AwareIM as a guest, no need for a login name or password. Be sure the guess accesslevel has access to your BO, the BO attributes and the your process.
MyInitProcess => change to your process name
MyInitObject => change to your BO
attr1 and attr2 => change to attribute names in your BO
@ => you can have more than 2 attributes, just put @ between the name=value pairs
field1 and field2 => these can be anything, just make sure they are the same in the input and oninput clauses. IMO I would use your attribute names
BSV => change to your BSV
Label1 and Label2 => change to what makes sense for your form
