Parent and Child Object Issues

I'm using v7.1 build 2223. I'm assuming I'm doing something wrong. This should be so straightforward, for the life of me I can't figure out why it won't work.

I have a Questionnaire Object and an Applicant object. A questionnaire object can have multiple Applicant objects.
On my questionnaire form, I have a few questions and then a table that lists the applicants and a Add New Applicant button that does an ENTER NEW Applicant WITH Applicant.Questionnaire=Questionnaire.

This isn't working. It doesn't seem to matter how I setup the questionnaire object initially. What I see in the log is that the id sent to the Applicant object is always -1.

The questionnaire object is in a popup that is launched from a menu item as a Create Object command.
When the Add button is clicked, it launches a process with the Enter New Command above.

I've tried Add New Reference, that still sends an id of -1.
I've tried Add new object, that fails.
I had a control that saved the form on a change so that I got an id for the Questionnaire object. But still when I hit add applicant the id it received is -1.
I've created a button on the main questionnaire form outside the applicants table that starts the process, that doesn't work. ID always -1.

What I need to do is so simple. Let the user enter the information and add as many applicants as possible. After each added applicant, the table on the main form should be updated. But this isn't happening. I hope that I'm missing something simple, but I feel like I've tried everything. I've done this on other projects and it worked so I'm not sure what I could've missed at this point.

Is there any particular reason why you use ENTER NEW in a called process rather than Add Reference?

If so, have you set Questionnaire as a required parameter for the Process?

ENTER NEW was what I was familiar with the last time I did it, so that's why I initially went that route.

I saw Add Reference and have tried it, but am not getting any success. How should I be using it? Does the parent object have to be saved before Add Reference would work.

Here's what I've done when I tried Add Reference
On the Questionnaire form, I have the Applicants table. I put a Add button with Add Reference in the top panel toolbar of the Applicants Table.

I'm going to try it again right now.

Oh yeah the Questionnaire was the value for input for the ENTER NEW process as well.

Just retried it with Add Reference. When I hit CREATE on the Applicant Child Object I get the Value of Questionnaire must be provided error

I got it to work. The parent object Questionnaire was set as required in the Applicant child object. So I guess it was hitting the required rule before it properly set the reference.

So I used Add Reference and it works well. Just need to make sure that the parent object isn't required in the child object first.
