Buttons remain on Edit Form After Delete

Hello,
I have an Edit form and I add a button to the Top of The Toolbar to see how it looked.
I proceeded to move the location of the button from the Top to the Right, Left then Bottom.
After seeing how it looked in each location I decided to keep the button on the right side.
After saving the form and going back in to this button still shows on the top and bottom of the form.
I have looked every where I can think of but cannot find where the button is in the list of buttons. It just show one button on the right side on the tool bar.

get rid of all buttons and add again afresh.
sometimes there's a glitch.
and sometimes you can delete all buttons, but one or more still appear.
and sometimes you can delete a button from a toolbar via the main dialog, and it appears to delete, but still appears. in this case you need to go into 'Edit toolbar' to delete the button.

I did delete all the buttons but will try doing it via edit toolbar. Then see if they are gone. If they are then I can add them back to the right side.

Click you button entries until the "Edit Toolbar" button is enabled.....

Click that the make sure no buttons are lurking in there..... That is where I found my problematic ones
