My application includes a lengthy questionnaire. Each question is an attribute, generally plain text with two options, yes and no. I simply changed the display properties (the prompt) to be the question being asked, and I used radio buttons as the option display. You say this did not work, but I'm curious why not. Nothing custom in my form.
It would also be possible to setup columns on your form and have an html cell for the prompt and an attribute cell for the response. You said you tried this too and I imagine this is not optimal anyway.
You say you couldn't get the attributes to line up. Do you mean horizontally or vertically? Horizontally, I presume. If you want the response to be right next to the prompt in every case, then try this...
Put in a separator for each question and set the width of the prompt to give you just the space you need. This will take for vertical space and may present some other unwanted design characteristics, but will provide maximum control.
