It is just a form operation showing above the form with the button image applied to the 'Display icon' image and 'Display text' unchecked.
I use this approach quite extensively on forms and queries as it provides very clear and noticeable calls to action for operations/processes.
It appears to me that the issue is being caused by the new styles. The table cell width for the operation bar that is now defined by the new vertical separators in the new gradient bar seems to be defaulting to a small width and then determined by the amount of text in the operation label. Since I don't use the label, the width is too small for the buttons. Looking at the compiled html source code from the browser, it looks like the width of these cells is now hard-coded to 10 pixels.
Also, how do I make the gradient bar above the form the same colour as the background as in my case, it provides a much cleaner look especially when the browser width is set to a greater width than the form width?
